On this episode of Advances in Care , host Erin Welsh and Dr. Craig Smith, Chair of the Department of Surgery and Surgeon-in-Chief at NewYork-Presbyterian and Columbia discuss the highlights of Dr. Smith’s 40+ year career as a cardiac surgeon and how the culture of Columbia has been a catalyst for innovation in cardiac care. Dr. Smith describes the excitement of helping to pioneer the institution’s heart transplant program in the 1980s, when it was just one of only three hospitals in the country practicing heart transplantation. Dr. Smith also explains how a unique collaboration with Columbia’s cardiology team led to the first of several groundbreaking trials, called PARTNER (Placement of AoRTic TraNscatheteR Valve), which paved the way for a monumental treatment for aortic stenosis — the most common heart valve disease that is lethal if left untreated. During the trial, Dr. Smith worked closely with Dr. Martin B. Leon, Professor of Medicine at Columbia University Irving Medical Center and Chief Innovation Officer and the Director of the Cardiovascular Data Science Center for the Division of Cardiology. Their findings elevated TAVR, or transcatheter aortic valve replacement, to eventually become the gold-standard for aortic stenosis patients at all levels of illness severity and surgical risk. Today, an experienced team of specialists at Columbia treat TAVR patients with a combination of advancements including advanced replacement valve materials, three-dimensional and ECG imaging, and a personalized approach to cardiac care. Finally, Dr. Smith shares his thoughts on new frontiers of cardiac surgery, like the challenge of repairing the mitral and tricuspid valves, and the promising application of robotic surgery for complex, high-risk operations. He reflects on life after he retires from operating, and shares his observations of how NewYork-Presbyterian and Columbia have evolved in the decades since he began his residency. On today’s episode of The Hardcore Therapist, I have the absolute pleasure of sitting down with Paulo Segadaes—better known as Sega. Sega is a powerhouse of creativity, blending his talents as a musician, cinematographer, and photographer into a truly inspiring career. Sega’s musical journey began as the drummer for Human Beans , but his hardcore roots were firmly planted when he joined the straight-edge band X-Acto from 1994 to 1999. After that, he explored new sounds with Sanyasse from 1999 to 2002, before becoming the drummer for The Vicious Five from 2003 to 2009. He later picked up the guitar as a member of Men Eater from 2009 to 2011, and from 2016 to 2019, he brought his rhythmic energy to The Legendary Tiger Man as their drummer. But Sega isn’t just a musician—he’s also a gifted cinematographer and photographer. His latest project is a stunning photography book capturing the vibrant spirit of Vrindavan, India. instagram handle @segadaes book - https://goodluckproducts.bigcartel.com/product/vrindavan-book…

On this episode of The Hardcore Therapist , I tackle a powerful question: What is a dismissive-avoidant attachment style, and how does it show up in relationships? A dismissive-avoidant person’s attachment style is shaped by a deep discomfort with emotional closeness and dependence. This isn’t something they consciously choose. More often than not, it’s rooted in their early experiences—where they may have learned that relying on others for emotional support wasn’t safe or dependable. To protect themselves, they developed a coping mechanism built on fierce independence and emotional distance. While this approach may have served them in their formative years, it can create significant challenges in romantic relationships, friendships, and even parenting.…
